Our Pittsfield client is seeking a Case Manager to support families, seniors, individuals with disabilities, and high-risk residents. The goal is to help residents live independently by connecting them with supportive services and integrating them into the broader community. This role works closely with leasing staff, Tenant Relations Specialists, and outside service providers to promote resident stability, independence, and well-being.
Key Responsibilities

Conduct assessments and develop individualized service plans

Provide case management and make appropriate referrals

Build relationships with service providers and monitor resident progress

Identify service gaps and coordinate solutions

Organize community integration activities and social events

Maintain accurate documentation and reports

Support property staff with tenant compliance and outreach efforts

Design and distribute community newsletters

Work independently across various residential and community locations

Stay current with HUD, MHFA, and other regulatory training (36 hours required in first year)
Qualifications

High school diploma or GED (Associate’s degree preferred)

2–4 years of experience in case management or human services

Strong interpersonal and communication skills

Comfortable with Word, Excel, and general technology

Valid driver’s license and reliable transportation
