CARE COORDINATOR

Our Pittsfield Client is hiring! If you have an interest in Human Services, or are a natural advocator, then a Care Coordinator position might be for you!  

This is a temp to hire, Full Time, 37.5 hour work week, Monday through Friday, 8a-4p. This organizations offers amazing benefits upon hire- including: Health/Dental/Vision Insurance, LT/ST Disability, Tuition Reimbursement, Life Insurance and 7 weeks of paid time off annually!


Care Coordinators should have some sort of experience or familiarity with Mental Health or substance abuse. Each Care Coordinator is assigned individual clients with whom they will meet (In person, Zoom, or Phone) monthly. Duties include working collaboratively with clients to create Care Plans of Goals for their client, charting visits, assisting clients with completion of online applications for various forms of assistance.
