The Quality Facilitator is primarily responsible for ensuring the delivery of quality supports and services. Responsible for oversite of ensuring regulations are being met as well as adherence to CARF/QUEST standards. The QA Facilitator performs program visits to assess all activities, programs and plans to determine if current services support BFAIR’s mission and values, reflect national accreditation standards as well as DDS’s vision and mission. These areas including implementation strategies for respect and dignity, individual control, community membership, relationship building, growth and accomplishment, and personal well-being for each individual served. The Quality Facilitator will monitor the quality of service delivery through the implementation of each person's Individual Support Plan (ISP).
QUALIFICATIONS: Bachelor's Degree and a minimum of 3 years’ experience delivering programmatic supports for individuals with disabilities required. Ability to complete all initial and ongoing background checks including but not limited to: CORI, SORI, Fingerprinting, LEIE, Driving record, E-verify, Physical, PPD and licensing and/or certification checks (i.e. nursing). Valid US driver’s license and ready access to a vehicle.
TRAINING REQUIREMENTS: Must maintain all certification needed in the program including: CPR, First Aid, MAP and BFAIR Human Rights Training. May need to attend workshops, seminars or BFAIR functions as needed for job development.
