The Community Health Worker Food Coordinator (CHW) will provide administrative support, delivery of food bags, and community outreach to CHP determined sites across Berkshire County. This person will support CHP’s mission, vision, and values and will adhere to compliance protocols as well as CHP’s policies and procedures.

Community Health Worker Food Coordinator Essential Duties and Responsibilities:

Administrative Duties:

  • Communicates with all sites regarding delivery days, times, and logistics.
  • Keeps records of each site, number of bags of food given and demographics.
  • Provides and maintains links to CHP database of nutrition education.
  • Orders supplies.
  • Prepares bags of food for each site.
  • Works with Berkshire Grown and Berkshire Bounty regarding logistics of delivered food from farmers.
  • Other duties as needed.

Traffic Control and Site Set Up:

  • Provides direction to patients to have optimal flow when picking up bags of food.
  • Sets up, tables, educational items, and food bags.

Outreach:

  • Ability to speak with residents and patients with confidence regarding food bags from local farmers.
  • Knowledge of SDOH programs and referrals to SDOH programs.
  • Ability to communicate and coordinate with practice managers at each CHP medical/dental site and Mobile Health Units.
  • Work side by side with Mobile Health Unit distributing food bags.

Distribution of Food Bags:

  • Ability to drive a refrigerated van to all CHP distribution sites in Berkshire County.
  • Must have a valid driver’s license.
