School-Based Case Manager - Youth Substance Abuse Services

Weekdays, 1st Shift! Full and Part-Time

This position will provide intensive case management to youth identified to be at risk within identified schools in the community.

Essential Job Functions

· Provide intensive case management

· Conduct case management meetings with parents/caregivers as well as personnel from school,

law enforcement, other stakeholders & youth as appropriate

· Perform intake interviews with youth and family as well as needs assessments and individual

service planning for youth as well as obtain and maintain appropriate releases of information.

· Maintain proper and complete documentation of meetings, releases, communications, referrals

and other essential paperwork in a timely manner as required by Brien Center standards

· Conduct Family meetings to ensure good communication between school, family and Brien

Center as well as phone calls to family on monthly basis at a minimum to keep updated on

progress

· Meet with Youth at a minimum or 1 time a week or as needed given circumstances surrounding

youth. Teaching skills to students, including but not limited to self regulation and executive

functioning skills

· Provide advocacy as well as make referrals to appropriate resources to assist youth and family

· Be available during school hours with some flexibility to include some late afternoon & evening

activities

· Provide Crisis on-call 24/7 for youth and families assigned within program

· Collaborate with Brien Center Clinicians as well as clinicians within Community working with

youth and family

· Identify and plan family events to bring youth and family together in a positive environment

· Attend meetings in community with Clinical or State agencies (DCF,DMH) involved with family

· Attend Brien Center mandatory trainings as well as trainings identified to be effective in working

with youth and families (ACRA, Motivational Interviewing, Harm Reduction, Risk and Protective

Factors, Strengthening Families, Nonviolent Crisis Management)

· Collaborate with the Brien Center Clinicians concerning substance use assessments and

treatment

· Participate in all relevant staff meetings, supervision and required trainings

· Keep supervisor well informed and update collateral services when necessary

· Report incidents regarding accidents, injuries and unusual events

· Be available to transport youth as needed

Qualifications

· BA in related field

· Experience working with adolescents and/or ability to establish and maintain effective

relationships with adolescents

· Training and/or experience in substance abuse or counseling and education
