Emergency Department Social Worker II- **SIGN ON BONUS AVAILABLE**

The Emergency Department (ED) Social Worker coordinates and facilitates discharge or admission planning in collaboration with the ED Case Manager and the health care team. The ED Social Worker will document their psychosocial assessment in the patient record and assist with transition to the inpatient units or community setting.

Schedule:

  • 40hrs per week
  • Hours: 8:00am-4:30pm
  • Rotating Weekends and Holidays
  • Experience:

      • 2 years of experience with a chronically ill population required. Background with all age populations preferred.

      • 2 years of experience in the assessment and engagement of individuals with Behavioral Health/Substance Use Disorder conditions required

      • Experience working in a healthcare setting as a member of a professional clinical team required.

      • Knowledge of community resources, Behavioral Health/Substance Use Disorder treatment as well as experience in referral for treatment, support and services is required.

      • Experience in providing evidence-based brief interventions to individuals with Behavioral Health/Substance Use Disorder conditions including Motivational Interviewing, Behavioral Activation, CBT and Problem Solving .


  • Education and Training:
        • Bachelor's Degree in Social Work or related clinical healthcare field required. Master's Degree preferred
  • License, Certification & Registration:
      • LSW- Licensed Social Worker required

      •  LCSW preferred
  • Other Requirements:

        • Ability to form rapid engagement and alliance building with patients and their families.

        • Ability to follow the logical management throughout the continuum of care.

        • Possesses a basic understanding of applying clinical criteria when determining medical necessity.

        • Proven excellent leadership, strong interpersonal, communication and team-building skills.

        • Proven collaboration & negotiation skills with providers, patients, families, insurance organizations and government agencies.

        • Proven ability to work independently and adjust to multiple demands and shifting priorities, ambiguity and rapid change with resiliency.

        • Computer literacy required with ability to manage multiple open applications.

        • Ability to maintain and monitor confidentiality of patient records.

      • Ability to work autonomously and with meeting goals for productivity.
