 The goal of the AFC program is to allow program eligible individuals with disabilities the opportunity  to live at home, with their families, or with community members/caregivers with appropriate medical and social services need met.  You will work with AFC participants, caregivers, and AFC care managers and RNs to plan, monitor and develop a plan of care, provide ongoing support and maintain proper documentation for each AFC participant.

Some Job Duties include:

  • Develop and review each member’s AFC plan of care on an ongoing basis.
  • Conduct on-site visits to all caregiver homes at least monthly for Level 2, and bi-monthly for Level 1.
  • Monitor the health status of all members and ensure that all necessary care is provided in the manner prescribed.
  • Complete a nursing progress note corresponding with each on-site visit or encounter, or more often as the member’s condition warrants.
  • Review caregiver logs on each visit to the AFC caregiver home.
  • Provide AFC caregivers health teaching/coaching on relevant health, aging, medical and special needs topics.
  • Act as an intermediary with the member’s physician and report any changes in health status or need for review.
  • Participate in the recruitment, screening and supervision of AFC caregivers in conjunction with each assigned AFC Care Manager.
  • Plan and collaborate with the implementation of discharges from the AFC program.


  • Licensed LPN in good standing with the Massachusetts Board of Registration in Nursing. 
  • Recent experience in the direct care of elderly or disabled adults is preferred.
  • Knowledge of community social services, education and health service agencies preferred.
  • Basic Microsoft Office Word and Outlook skills.
  • Comfortable with remote application such as Zoom for team meetings as needed.
  • Ability to work flexible hours to meet the needs of AFC families.
  • Negative Mantoux TB test or a clear chest x-ray
  • Must have an acceptable CORI, SORI, DPPC and National Fingerprint Check.
  • Valid driver’s license, availability of private vehicle, up-to-date car insurance.
