The goal of the AFC program is to allow program eligible individuals with disabilities the opportunity  to live at home, with their families, or with community members/caregivers with appropriate medical and social services need met.  As an AFC nurse, you will provide ongoing support to AFC participants, caregivers, and AFC care managers.  Responsibilities include but are not limited to

  • Develop and ensure ongoing review of each member’s AFC plan of care.
  • Complete nursing progress notes corresponding with each on-site visit or encounter, or more often as the member’s condition warrants.
  • Review caregiver logs on each visit to the AFC caregiver home.
  • Complete clinical assessments 
  • Monitor the health status of all members and ensure that all necessary care is provided in the manner prescribed.
  • Serve as an intermediary with the member’s physician 
  • Participate in the recruitment, training and support of AFC caregivers in conjunction with each assigned care manager on all matters of health, aging, medical conditions, and special needs
  • Conduct monthly or bimonthly on-site visits to caregiver homes
  • Discharge planning
  • Track hospitalizations of AFC members.
  • RN/BSN
  • Massachusetts Licensed Registered Nurse 
  • At least two years’ recent experience in direct care of the elderly or disabled adults.
  • Specific skill- based competencies are required to satisfactorily perform the functions of the job; these include analyzing data, effective listening, and communication skills, facilitating meetings, monitoring activities, planning and problem solving.
  • Basic computer skills, working knowledge of remote communication such as Zoom.
  • Valid driver’s license, availability of a private vehicle, up-to-date car insurance and proof of annual vehicle inspection.
  • The ability to work flexible hours to accommodate the needs of members and families.
  • Satisfactory CORI, SORI, DPPC, and National Fingerprint Check.
  • A negative Mantoux TB test or a clear chest x-ray.
