Under the direction of the direction of the Director of Family Support & Advocacy the Licensed Practical Nurse (LPN) will be responsible for the assessment, planning, and monitoring of individual health needs with the support of a families assigned Registered Nurse (RN). The nurse is part of the Adult Foster Care Program multidisciplinary team and works directly with The Director of Adult Foster Care Program.

Under the direction and supervision of the Director of Advocacy & Family Support per the AFC Regulations the licensed practical nurses may:

  • complete and coordinate all applicable clinical assessments and clinical evaluations;
  • develop and review on an ongoing basis each member’s AFC plan of care;
  • provide oversight to AFC caregivers in conjunction with the care manager;
  • review the PCP Summary Form;
  • monitor the health status of all members to ensure that all needed AFC is properly delivered;
  • report changes in the health status of any member to the member’s PCP;
  • review AFC caregiver logs at a minimum of every 30 days;
  • conduct on-site visits with each member at the qualified setting in accordance with 130 CMR 408.415(B)(6);
  • complete a progress note corresponding with each on-site visit or encounter, or more often as the member’s condition warrants;
  • submit semiannual health-status reports to the member’s PCP;
  • plan for and implementing discharges and transition from the AFC program;
  • conduct an orientation for each AFC caregiver before the AFC caregiver begins personal care; and
  • provide ongoing training to AFC caregivers on health and aging.
  • Licensed Practical Nurse with four years’ Two years’ experience in with ID/DD populations or the elderly. Current license in the State of Massachusetts.
  • Valid driver’s license and insured vehicle.
