The Nursing Home Transition Specialist will identify and assist people with disabilities to move from institutions to community based settings. The position involves extensive contact with nursing home professionals, consumers, family members, and other human service providers. Work includes teaching consumers how to access benefits and services such as health insurance, housing, social security, and food stamps.

This position will need to be able to learn and utilize MassHealth waiver programs specifically designed to help people transition out of institutions. In addition, the NHT Advocate will provide mentoring, skills training, and information & referral services.


This position is full-time 40 hours (1 hour paid lunch daily) and 75% of work takes place in the Pittsfield office or in the community/nursing homes. This position requires travel with personal vehicle, home visits (ascending/descending stairs), electronic and written documentation, and ability to understand benefit rules and regulations. Primary working hours are 9am-5pm Monday through Friday.
