SEEKING: We are seeking a Case Manager for our Supportive Services Program. This position will be located in North Adams, MA.
PROGRAM DESCRIPTION:
The Supportive Services Program provides comprehensive early education and family support for children and families who are funded by the Priority Population childcare contracts. Services include child care in either a Family Child Care (FCC) home or center, transportation, assessment of delays, Individual Family Service Plans and connections to community resources, with the goal of positive developmental progress, school success, reducing child maltreatment, parent resilience and strengthening the family.
BASIC FUNCTION:
The Case Manager works with children and families receiving child care through the EEC priority population and income eligible contract to support the individual needs of children and families primarily in Center Based programs, but may also include children/families in Family Child Care (FCC) depending upon caseload variability.
The Case Manager will assist in the creation, support, and monitoring of Individual Family Plans, make and follow up on service referrals, crisis intervention, reporting and documentation of children’s records. They will provide ongoing support to families regarding child behavioral success plans, as needed. The Case Manager maintains communication with DCF caseworkers concerning children’s progress and any emerging family issues. In addition, offers educators support concerning children’s success in placement. He/she informs Program Director and Supportive Services Program Director of any problems, or gaps in service. The deliverance of high quality, comprehensive, early education services that meet the individual needs of the child and promote educational success is the primary goal.
