BHS is recruiting dedicated LPN to complement their outpatient primary care team in North County as a champion for patient wellbeing and engagement.

The Population Health Nurse works as an integral part of an interdisciplinary team to conduct, coordinate and complete the non-provider portions of preventive wellness services such as Medicare's Annual Wellness Visits. Other priorities inlcude: 

  • Assists with population health management through patient outreach and coordination of care, participates in the clinical visit to assist the provider, and provides self-management support to patients.
  • Demonstrates leadership by communicating effectively, developing others, building and maintaining relationships, and guiding effective teams and work groups.
  • Mentors others within the team with office functions to support patient care and office operations. Follows all established protocols, policies, procedures and standardized work flows.

POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.)

  • Experience:
    • Has a thorough knowledge of nursing principle and procedures.
    • Experience in handling multiple telephone lines and excellent clerical/organizational skills required.
    • Experience in specimen testing and physician office experience preferred.
  • Education and Training:
    • Graduate of an accredited School of Practical Nursing.
  • License, Certification & Registration:
    • LPN - Licensed Practical Nurses
    • CPR
  • Other Requirements:
    • Demonstrates continuing education in accordance with State regulation.
    • Data entry skills preferred.
    • Working knowledge of Windows and PC applications required.
    • Ability to work independently and take direction.
    • Self-motivated and has the ability to be an independent and flexible in a team framework.
    • Excellent oral and written communication skills.
    • Demonstrates ability to handle confidential and sensitive information.
