Temporary through March 2025 Registered Nurse Case Manager

Responsibilities include determining clinical eligibility for MassHealth-funded services for nursing facility stays and completing in-home assessments of service needs for individuals. Excellent written, communication, and computer skills required. Geriatric/Long Term Care and community-based nursing experience preferred. Registered nurse with a valid Massachusetts license. Bachelor of Science degree in Nursing or Associate of Science in nursing and at least one year of clinical experience in a community or long-term care setting.

· Ability to develop and maintain records and communicate clinical data in verbal and written form

· Demonstrated knowledge of the principles and practices of nursing

Expected hours: No more than 30 per week
