Under the general supervision of the Administrator and Director of Nursing, the assigned Certified Nurse's Assistant (CNA) will be responsible for documenting and maintaining medical records required for participants in the Restorative Program. The CNA will also coordinate with all other facility CNAs, Nurses and Rehab Team members to support each resident's ongoing mobility goals. The Restorative Aid will: · Assist with identifying and preventing high risk resident decline. · Develop attainable goals with each resident/family member to maintain baseline level of functioning upon discontinuation of rehab services. · Audit the residents at high-risk for falls and develop strategies to prevent falls. · Provide monthly fall prevention audits and check to ensure interventions for each resident are in place (floor mats, alarms etc...) · Review medical equipment for the safety, integrity, and appropriateness of each resident. · Assist CNAs with meeting mobility goals. · Participate in Med B / CMI rounds and Risk meetings weekly. · Provide monthly updates to the Director of Nursing on each resident's progress · Accurately and timely document restorative minutes. |
