Berkshire County Arc is a non-profit human service agency that has been providing community-based services to adults with developmental disabilities and/or brain injuries for over 50 yrs. $1,500 Sign On

Mon-Fri    40 hrs/wk

Responsibilities:

  • Serve as primary contact person and coordinator of medical supports for all assigned individuals
  • Advise the program director and other DH provider staff of any medical problems that may hinder a member’s participation in DH or in a specific activity. Keep Program Director informed of changes in health and status of all individuals.
  • Ensure that all paperwork is up to date, including annual physicals, doctor’s orders, ER folders, medical protocols, daily notes, monthly notes, semi-annual and biennial notes, etc.
  • Complete all required documentation on individuals assigned, including day habilitation and HCSIS requirement
  • Complete nursing assessments
Requirements:
  • Licensed Practical Nurse with one year experience. Some experience with people with intellectual or developmental disabilities or acquired brain injury is ideal. Current license in the State of Massachusetts.
  • Valid driver’s license and insured vehicle
