Company Description:Berkshire County Arc is a non-profit human service agency that has been providing community-based services to adults with developmental disabilities and/or brain injuries for over 50 yrs. Berkshire County Arc’s mission is to assist and support individuals in their quest to identify and realize their chosen lifestyles. This is accomplished through various programs offered: residential homes including independent apartments, employment services, day programs, adult family care, transitions, advocacy & family support, special needs pooled trusts and a mail-house staffed by people with disabilities that serves dozens of businesses across Western Massachusetts.
Berkshire County Arc’s programs are offered in Berkshire County and Pioneer Valley. We serve nearly 1,000 individuals per year and have over 700 dedicated staff.

Description: | $1,500 Sign On Mon-Fri 40 hrs/wk Responsibilities: - Serve as primary contact person and coordinator of medical supports for all assigned individuals
- Advise the program director and other DH provider staff of any medical problems that may hinder a member’s participation in DH or in a specific activity. Keep Program Director informed of changes in health and status of all individuals.
- Ensure that all paperwork is up to date, including annual physicals, doctor’s orders, ER folders, medical protocols, daily notes, monthly notes, semi-annual and biennial notes, etc.
- Complete all required documentation on individuals assigned, including day habilitation and HCSIS requirement
- Complete nursing assessments

| Requirements: | - Licensed Practical Nurse with one year experience. Some experience with people with intellectual or developmental disabilities or acquired brain injury is ideal. Current license in the State of Massachusetts.
- Valid driver’s license and insured vehicle
