Description:

Community Living Options Coordinator
Job Type: Full Time
Schedule: 37.5 hours/week (M-F)
Location: Pittsfield, MA
$1,500 Sign-on Bonus!

Coordinator Position Overview:
The Community Living Options Coordinator completes administrative tasks relating to the Community Living Options departmentor in support of the overall agency. The goal of this role is to provide help that will allow UCP staff to manage their time mosteffectively and positively represent the agency to members, staff, and visitors. In addition, this role aids in the process of creating andfinalizing Transitional Assistance Plans and Housing Search Entity Plans in conjunction with the Assistant Director of CommunityLiving Options.

Coordinator Responsibilities:

  • Organize and maintain physical and electronic documentation.
  • Scan, upload, and photocopy documents.
  • Respond to general inquiries and distribute messages.
  • Manage communications between departments, agency staff, and external partners.
  • Attend meetings, take minutes, and assist with special projects as requested.
  • Handle housing search referrals and documentation submissions.
  • Maintain member information, handle billing, and ensure compliance with policies.
  • Maintain certifications, comply with regulations, and participate in training.
  • Assists with Housing Search Entity Referrals and submission of documentation in conjunction with Assistant Director ofCommunity Living Options and Supported Living Care Navigators.
  • Comply with the rules and regulations stated in the Provider Manuals for the ABI/MFP Waiver programs.
  • Provide direct support, transportation, and on call services on an as needed basis.
  • Participate in conferences and other in-service training as requested.

Requirements:

Coordinator Qualifications

  • A high school diploma or equivalent
  • Proficient in computer skills, MS Office Word, Excel, Access, and PowerPoint with ability to learn new systems and software.
  • Must possess excellent typing and data entry skills.
  • Ability to work flexible hours as needed.
  • Able to obtain and maintain CPR Certification
  • Knowledge of local community resources and community-based providers, and UCP programming
  • A valid driver’s license, proof of insurance and the availability of a private vehicle
  • Satisfactory CORI, SORI, DPPC, National Fingerprint background checks.
  • Prior knowledge and experience with navigating housing applications including subsidized and low-income housing is preferred.
