Description:
<p>The Quality Facilitator is primarily responsible for ensuring the delivery of quality supports and services.&nbsp; Responsible for oversite of ensuring regulations are being met as well as adherence to CARF/QUEST standards. The QA Facilitator performs program visits to assess all activities, programs and plans to determine if current services support BFAIR&rsquo;s mission and values, reflect national accreditation standards as well as DDS&rsquo;s vision and mission.&nbsp; These areas including implementation strategies for respect and dignity, individual control, community membership, relationship building, growth and accomplishment, and personal well-being for each individual served. The Quality Facilitator will monitor the quality of service delivery through the implementation of each person's Individual Support Plan (ISP).</p><p></p><p></p><p>&nbsp;</p>
Requirements:
<p><strong>QUALIFICATIONS:&nbsp;&nbsp;&nbsp;&nbsp; </strong>Bachelor's Degree and a minimum of 3 years&rsquo; experience delivering programmatic supports for individuals with disabilities required. Ability to complete all initial and ongoing background checks including but not limited to: CORI, SORI, Fingerprinting, LEIE, Driving record, E-verify, Physical, PPD and licensing and/or certification checks (i.e. nursing).&nbsp; Valid US driver&rsquo;s license and ready access to a vehicle.</p><p><strong>TRAINING REQUIREMENTS: </strong>Must maintain all certification needed in the program including: CPR, First Aid, MAP and BFAIR Human Rights Training.&nbsp; May need to attend workshops, seminars or BFAIR functions as needed for job development.</p>
