| <p>Do you want to assist seniors and individuals with disabilities in an interactive, decision making process to help them identify community resources? Options Counselors work with the individuals and their family members to connect them to vital resources and services that fit their current situation and preferences to stay in their home. Work collaboratively with clients, families and community resources. </p><p>Options Counselors guide individuals through their questions about living in the community:</p><ul><li>Can I live in the community safely and independently?</li><li>What services and supports are available for me in my home?</li><li>Can I go to a rehab facility or nursing home temporarily and then return to my home?</li><li>What services, assistive devices, or home modifications are available to support me in my home?</li><li>Will my insurance cover the costs, and if not, what funding resources may be available for services and supports?</li></ul><p>Strong oral and written communication skills. Bachelor’s degree required. Relevant experience in senior and/or disability services preferred. AA/EOE</p> | |
