Job Type: Part Time/Per Diem Supported Living Adult Berkshire County- Northern, Central and Southern Position Highlights: Are you a Personal Care Assistant looking to make a difference? The role of the UCP Supported Living Adult Companion/PCA position is to work with members to maintain and strengthen daily living skills through assistance and supervision of daily tasks and assistance with personal care needs. This service is provided in accordance with a therapeutic goal in the service plan. We are looking for qualified individuals located in or near all areas of Berkshire County. Duties will include but are not limited to: Interact appropriately and teach positive interaction and social skills. Facilitate and coordinate appointments (medical, dental, mental health, etc.) in conjunction with the Surrogate (if applicable). Address individual’s needs by the goal and objectives within the Supported Living service plan. Perform light housekeeping tasks and some personal care needs that are incidental to the care and supervision of the individual. Transport member as needed for community outings, grocery shopping and/or medical appointments. Provide weekly reports to the Support Living Care Navigators. Comply with the policies and regulations stated in the Provider Manual for the MRC ABI/MFP Waiver and the MRC Supported Living In-Home Supports Manuals. | |

•High School Diploma or equivalent GED •Life or work experience with individuals with disabilities •Prior PCA Experience preferred •Ability to navigate crisis/emergency situations. •At least 18 years of age •Valid driver’s license, availability of private vehicle, up-to-date car insurance and proof of annual vehicle inspection •Ability to work flexible hours in order to accommodate the needs of members •Satisfactory CORI, SORI and National Fingerprint Check •A negative Mantoux TB test or a clear chest x-ray •Current annual physical Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the job.
