Homecare Service Coordinator
High School or Equivalent

PCA/Homemaker of PHCAST - Preference: Required
The Homecare Services Coordinator will develop, maintain and/or maximize the consumer’s independent functions. This position will provide support in self-care, emotional growth, socialization, communication, vocational skills as the objective is to improve health and welfare so the consumer can establish and/or maintain residency in the community. The Coordinator will also by responsible for Transitional Assistance functions designed to facilitate transition for consumers between a nursing facility/hospital to a community living arrangement this includes assistance with securing basic household items.
QUALIFICATIONS: A High School Diploma/GED with 1year life or work experience with individuals with ABI and/or disabilities. Certification as a Personal Care Worker/Homemaker of PHCAST. Ability to complete all initial and ongoing background checks including but not limited to: CORI, SORI, Fingerprinting, LEIE, Driving record, E-verify, Physical, PPD and licensing and/or certification checks (i.e. nursing). Valid US driver’s license and ready access to a vehicle.
TRAINING REQUIREMENTS: Must maintain all certification needed in the program including: CPR, First Aid, and BFAIR Human Rights Training. May need to attend workshops, seminars or BFAIR functions as needed for job development.
