To support the overall mission of the Adult Family Care (AFC) program and implement its philosophy and goals so that all levels of the organization, its activities, programs and plans recognize and utilize strategies to promote personal well-being for each member served.
Treat each individual, co-worker, supervisor and others with the respect and dignity due every human being, and adhere to the Department Developmental Services (DDS) Human Rights Standards and the Disabled Persons Protection Commission’s (DPPC) Mandated Reporter Guidelines around physical and emotional abuse, including the timely reporting of violations.
Facilitate quality services through adherence to agency, DDS, AFC, and DMA standards. Adhere to the Disabled Persons Protection Commission’s Mandated Reporter Guidelines.
Complete and sign the designed screening tool as required under 130 CMR 408.416 (C) for Mass Health Payment. Complete the MDS-HC or CDS and other nursing assessments and coordinate all other applicable clinical assessments. Review the physician summary.
Develop and review on an ongoing basis each member’s AFC plan of care.
Select, train, evaluate and supervise AFC caregivers in conjunction with the Care Manager.
Monitor the health status of all members to ensure that all needed AFC is properly delivered.
Report changes in the health status of any member to the member’s physician.
Periodic review of AFC caregiver logs.
Conduct on-site visits with each member at the qualified setting bimonthly (alternating with the bimonthly visit by the care manager) for Level I and monthly or more often as the member’s condition warrants for Level II.
Complete nursing progress note corresponding with each on-site visit or encounter, or more often as the member’s condition warrants.
Submit a semiannual health-status report to the member’s primary care physician.
Plan for and implement discharges from the AFC program.
Conduct an orientation for each AFC caregiver before the AFC caregiver begins personal care.
Provide ongoing training to AFC caregivers on health and aging and ensure other required trainings are met.
Adhere to agency/personnel and DDS, DPH and AFC policies. Follow emergency and on call procedures.
Participate in conferences, workshops and related trainings to stay abreast of developments in the field.
Perform other duties as assigned by the AFC Director and/or Administration.
QUALIFICATIONS: Must be fully licensed as a Registered Nurse by the Massachusetts Board of Nursing and have at least two years experience in the direct care of the elderly or disabled. Ready access to an automobile and a valid driver’s license also required.
Receive and maintain current certification in First Aid, CPR and BFAIR Human Rights Training, 10 hours professional development annually.
