Licensed Social Worker

Fairview Commons Nursing & Rehabilitation Center in Great Barrington is currently seeking a reliable and professional individual to join our team. Our Licensed Social Worker assists residents and their families in coping with the psychosocial and emotional issues related to illness and aging. Assists in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Work Department in accordance with current Federal, State, and Local standards, guidelines and regulations. Experience in short term rehab including admission and discharge planning strongly preferred.
•Must possess a Bachelor’s Degree, preferably in Social Work, and a current Massachusetts State license (LSW) to practice Social Work. Masters Degree preferred.
•Ability to assess needs from admission to discharge, develop treatment plans, and document interventions/information in medical records.
•Coordinate appropriate discharge planning options.
•Must be willing to make every effort to meet resident and family requests
•Must be willing to seek out new methods and principles and be willing to incorporate them into existing practices.
•Must have working knowledge of basic computer applications.
•Ability to interact effectively with residents, families and co-workers.
•Ability to communicate effectively, in writing, as well as, verbally.
•Ability to plan, organize, develop, implement, and interpret the facility department programs, goals, objectives, policies and procedures of the Social Work Department.
